A Saint Mary's College 旅行健康表格 is required for all SMC-sanctioned study abroad or Jan Term Travel. Completed forms must be provided to the Jan Term Travel office or 出国留学 office to enable travel.

的 Student Health Center offers travel appointments to complete this form. A visit fee of $25 will be charged to your account for these appointments. Alternately, this form can be completed by any off-campus healthcare provider of your choice. 

To Schedule your Travel 访问 at the Student Health Center:

Please include the following in your email:

     1) Subject heading of "Travel 访问"

     2)全名/SMC ID

     3) Travel destination and dates

One of our nurses will get back to you to review your immunizations. To expedite this process, please make sure documentation of your current immunization record (hopefully you uploaded this upon your enrollment!) has been uploaded to 地中海+普氏. (If you entered SMC before Fall 2023, please make sure your record was uploaded to the Student Health Portal OR you can upload to 地中海+普氏)

For travel paperwork to be approved by the SMC Student Health Center, all immunizations recommended by the 疾病预防控制中心 for your travel destination are required. We must take into consideration the health and experience of your entire travel cohort.  相应的, for forms to be signed by the Student Health Center staff, ALL Jan Term travel destinations will require the following:

     1) Routine childhood vaccines: MMR, 水痘, 小儿麻痹症, Tdap (within 10 years), Meningitis ACWY (after 16th birthday).  参阅疫苗详情 在这里

     2) Flu shot (available at the Student Health Center at your in-person visit)

     3) Covid-19 (evidence of initial series OR one dose since August 2022)

In addition to the above, the following will be required according to destination:

Brazil (4 additional) : 1) Hepatitis A (2 doses at least 6 months apart or 1 dose if travel is within next 6 months), 2) Typhoid vaccine (prescription will be given at in-person visit if needed), 3) Anti-Malarial medications (prescription will be given at visit), 4) Yellow Fever (Travel form can/will be signed prior to obtaining this, but it should be acquired 10 days prior to travel: http://wwwnc.CDC.gov/travel/yellow-fever-vaccination-clinics/search)

墨西哥, 尼泊尔, OR South Korea (2 additional) : 1) Hepatitis A (2 doses at least 6 months apart or 1 dose if travel is within next 6 months), 2) Typhoid vaccine (prescription will be given at in-person visit if needed)

Your in-person Travel 访问 will be scheduled once it is determined your pre-visit immunization requirements have been met. 

If anti-malarial medications or the typhoid vaccine are required for your destination, prescriptions will be given to you at your in-person travel visit. And as mentioned above, if a Yellow Fever vaccine is required, this may be obtained after your form is signed. (You can find vaccine clinics offering this 在这里).

Preparing for Your In-Person Appointment

What to bring when you schedule an appointment:

  • 旅行健康表格, with page 1 completed by the student.
  • any documentation of additional vaccines obtained after speaking with our travel nurse

We highly encourage all students to utilize our email-started travel visit (see above), as this will expedite the process. Travel appointments will not be scheduled unless approved by the emailing travel nurse.

Cancellation and No-Show Policy
